I think this is the case where Todoist really nails it. Their queries (which can be saved as filters) support
- include / exclude
- grouping ()
Ideally it would let you upload from desktop - my files aren't always on the cloud
Agreed. GTD encourages you to defer tasks to your calendar - these are not DUE dates, but dates before which you cannot start the task (Eg.., you can't talk to Dave until Dave is back in town next week).

even at $5/month, $60 is twice the cost of Todoist, and the cost of a full Google Drive subscription with lots of space. I'm happy to pay to support you but the price may cause a problem in the long term.
